Before submitting an appointment request, please acknowledge our No-Show and Late Cancellation Policy.

You must cancel or reschedule your appointment at least 24 hours in advance of your scheduled appointment time, or you may be subject to the following fees:

● New Consultations / Pre-paid appointments: $50.00
● All other appointments: $100.00

A grace period of 15 minutes will be provided. Any late arrivals without proper notice to our office will be considered a no-show or late cancellation. A courtesy call will be made to notify you of your terminated appointment. We will be more than happy to offer the opportunity to reschedule for the next available date and time.

We understand that life happens and plans change, so please call us at (312) 340-6740 to communicate any delays in your arrival so we can accommodate or reschedule!

When booking an appointment, you will be required to keep a card on file in the instance of a no-show or late cancellation. You will never be charged to book an appointment, only if in violation of the above outlined No-Show and Late Cancellation Policy.

If you do not wish to provide a card to have safely stored, you may opt to provide a non-refundable $50.00 deposit that will be applied to any treatment or procedure. This is only forfeited in the instance of a no-show or late cancellation. Your appointment will not be confirmed without one of these two methods.
